In our opinion, the new challenge of our profession as dentists is to remodel the lower part of the face (teeth, lips, cheeks, chin) to magnify a face and a smile, illuminated by healthy and clear teeth.

In our society of image, appearance, beauty according to the criteria of the new generation “Homo sapiens médiaticus”, it is fundamental not to neglect any of the most elementary tools to preserve our happiness of living. For it is our self-preservation instincts that guide us in resolving obstacles to our development.

A person whose body image is abnormally degraded cannot be in harmony with his social environment and therefore with himself. At all levels in the competition of life, both private and professional, this loss of opportunity is a source of drama and frustration. To find the image that corresponds to one’s spirit, and therefore to the one one wants to show to the world, is to find a dignity lost over time.

The odontologist who became the “Smile Designer ” of the XXIst century.

The “Smile designer” or “odontologist 2.0” has for mission to restore the harmony between body and mind in order to help people to love themselves and to be loved.

In this modern context, it is imperative to take into account the evolution of the traditional concepts of dentistry, which have been extended to the point of becoming intertwined and adjusted with the disciplines of their medical and cosmetic surgeons colleagues. But also, of course, he will have to join the unavoidable computer engineers, who are also the “designers” of all the odontological tools of the future, that is to say, our immediate present.

It will have to take into account new digital technologies, such as (but not limited to) 3D digital medical imaging with the Cone Beam Scanner and 3D virtual modeling of the maxillary and related structures. It cannot avoid acquiring the tools that are essential for modern clinical exploration and assistance in exhaustive diagnosis. Similarly, computerized surgical robotics can provide valuable assistance in the performance of surgery, thanks to preoperative planning or intraoperative navigation.

It will have to take into account the contribution of modern techniques of 3D modelling of prostheses made of dental ceramics, zirconia (or any other type of cosmetic material). It should be possible to produce high quality aesthetic ceramic prostheses with CAD/CAM systems (i.e.: CAD CAM) and allow an optimal “ad integrum” restoration of the smile and face, guaranteeing a more pleasant social and emotional life.

It will have to take into account the new techniques of global reconstruction of the dental arches (i.e. when the teeth are absent and the maxillae are resorbed) thanks to the modern protocols of global implant-supported prostheses (screwed onto the dental implants) made from 3D digitized anatomical models. That is, dentures that replace and mimic lost tooth and gum mass. It is to allow a plastic and reconstructive rehabilitation of the dental arches, essential to the functions of mastication, swallowing, as well as phonetic and body language.

It should take into account the management of sinus pathologies. That is to say, to add the skills of radiologists, maxillofacial surgeons and ENT surgeons who are now the main interlocutors. Their opinion is the essential prerequisite for pre-implant bone grafts, such as those in the bottom of the sinuses: sinus fillers.

It will have to take into account the tools resulting from the most innovative technologies. It means, for example, getting a “Piezotome”. An invention that gave birth to the concept of “Piezo-surgery”: an “instrument that cuts hard tissue but spares soft tissue”. Or, a “Laser” that finds new applications every day.

He will have to take into account the concepts of “biocompatible grafts” to carry out bone grafts intended for the reconstruction of bone crests or the filling of sinuses. It means offering patients less mutilating surgical alternatives and freeing them from self-grafts.

It will have to take into account the scientific revolutions and new operating protocols. It is an essential and permanent intellectual update on new treatment paradigms, such as dental care under sedation by narco-hypnosis, the placement of implants in immediate loading and aesthetics, or autologous platelet concentrates, PRP (Platelet Rich Plasma) or FRP (Fibrin Rich Plasma) These tissue healing boosters, which are unanimously accepted in high-level surgical fields (cosmetic surgery or orthopedics or dental implantology) are already integrated into all our dental surgical protocols. In the meantime, growth factors (Bone Morpho Protein – BMP2) and/or stem cells are readily available.

He will have to take into account the alterations of the patient’s psyche which goes hand in hand with the degradation of his body. The connections between our profession and psychology are so obvious that it is not even necessary to mention them, since the treatment of the deterioration of “self-esteem” is a corollary of our aesthetic results. Knowing how to refer to the science of psychologists and psychiatrists, specialists in stress, is also a component of our diagnosis.

He will have to take into account the management of the dentist’s fear and will have to call upon the skills of anesthesiologists (as any surgeon would) in order to sedate stomaphobic patients or those who are geographically far from specific treatment centers. It is to allow these patients, desperate because in medical wandering, to benefit from these treatments of reconstruction of the Body and the Spirit.

The corollary of the integration of all these new technological concepts is a reasonable objective of “Restitutio ad Integrum” of the smile aesthetics. But more than that, it is the aesthetic harmony of the entire lower face and thus of the healthy and beautiful expression of the individual altered by life, that the “Smile designers” reconstruct.

We are in line with the objectives of “Plastic, Reconstructive and Aesthetic Surgery”, but also of “Aesthetic Medicine”.

Everything is connected. Since we need to know how to manipulate new facial soft tissue fillers such as hyaluronic acid potentiated by PRP (Platelet-Rich Plasma) or PRF (Platelet-Rich Fibrin) in order to magnify lips and face.

All these new variables must be taken into account by the “Smile designer” and be integrated into the equation that governs our exercise in perpetual intellectual change.

Some people do not live like the average person: their pace of life, their professional responsibilities, their lack of physical and mental availability, cannot be reconciled with a time-consuming treatment of the dental damage that time has accumulated.

Darwin liked to say that: “the one who would survive would not be the strongest nor the most intelligent but the most adaptable”.

Therefore, we have adapted a team of “Smile Designers” to the requirements of these singular patients. We have provided them with sedation dental care protocols that allow them to compact all their dental treatment plans, even those of all types, including surgical. All sedations are performed by the anesthesiologists of our team.

It is thus in a shortest time, without any fear and without any operative pain, that we can welcome all kinds of complex personalities. But also all nationalities, even the most remote, for whom time is as scarce and precious as high-tech dental care.

OUR MEETING WITH DENTAL CARE PHOBIOS: stomatophobes

In dental surgery, the relationship between the patient and the dentist is essential.

In my personal life, I have often met people who say to me, “Oh my! Are you a dentist? What a horror! I will never go! I’m too afraid!”

However, the professional deformation of most dentists makes us unconsciously diagnose instantly the people we meet. As a matter of course, we see that many people have their teeth in poor condition and sometimes dramatically.

As studies have shown for a long time, rather than a possible questionable hygiene, many people suffer from an incoercible and especially unavoidable fear of dental care.

As a result, this harmful psychological state makes any comprehensive treatment of their physical (dental) and existential (disgrace) suffering unlikely.

These blockages, due to an abnormally intense stress in a dental office (stomatophobia), induce carious dental decay, often serious, both in terms of masticatory food function and social functions, but also emotional.

In order to help these patients access dental care and resolve their suffering, a technical platform for care under sedation has been set up with the cooperation of a team of anesthetists.

These sedation techniques (narco hypnosis or general anesthesia) adapted to dental care are borrowed from general and cosmetic surgery which have been practicing them for a long time.

Thus, all kinds of treatments can be performed without any fear or pain during the operation.
